Dentist Agreement Form

1. Applicant is responsible for paying taxes and social security. It is the final decision of the client (Doctor) whether full compensation is given or taxes are removed.
2. Dental Referral Systems is in no way responsible for personal injury or property damage. Applicant accepts all liability.
3. All referrals are the exclusive use of Dental Referral Systems. Any agreement with a client must be contracted through Dental Referral Systems. Applicant must notify Dental Referral Systems if he/she is referred to another employer who hires applicant in any capacity within two years from the last date worked or referred to original client, whichever is greater. No bartering is to be made with any client concerning future work.
4. If the applicant cancels a position after a job order has been completed there will be a $40.00 fee to applicant.
5. There is no fee to applicant for job referrals permanent or temporary.
6. Applicant will be paid $450-$600 dollars a day, depending on experience.
7. Applicant must notify Dental Referral Systems if he/she is re-employed or extends job order originally requested within two years of last date worked or referred to client whichever is greater, applicant must notify Dental Referral Systems of conversion.
8. If a temporary position converts to permanent employment within one year of the last date worked or referred to client, whichever is greater, applicant must notify Dental Referral Systems of conversion.
9. If the position fails for any reason the salary will be paid up to the last day of employment.
